After completing this help guide, you’ll be able to:

  • Create the components required for an Item Survey
  • Assign a Survey to an item


Surveys can be added to items (registration and store items) for users to fill out when purchasing items. Surveys are made up of three components,

  1. Questions
  2. Groups
  3. Templates



To Create Questions:


After selecting Questions in the Item Surveys menu, you will be taken to a screen with a list of all questions under two different tabs, Active and All. The Active tab will show all currently active questions, and the All tab will show all active and inactive questions.


Here you can select Add to create a new question, or select an existing question from the list to edit.



After selecting Add, you will need to fill in the following details to create a question,

  • Name - this is the question you are asking
  • Data Type - choose between,
    • Integer
    • Decimal
    • Text
    • Date
    • Date & Time
    • Yes/No
    • List
      • You can input your own list in the entries box, e.g 1. Confident, 2. Somewhat Confident, 3. Not Confident at All. Hit enter on your keyboard after each entry to submit.
    • File
  • Instructions - the description shown to the user answering the question
  • Display type - how the instructions are displayed,
    • Hint - instructions will show when mouse hovers over the information icon
    • Note - instructions will show under the question
  • On/Off Switches
    • Show Label
    • Mandatory - if the question is compulsory or not
    • Active/Inactive
  • Description - this description won't be viewed by users answering the question, it is for your use to describe the question

Once the fields have been filled in, select Save.



Once the question has been saved, you can use the Configurations tab to make text customisations - this is not compulsory and questions can still be used without filling in the configurations details.

  • Text colour

  • Text decoration


If you have selected 'Date' or 'Date & Time' as your data type for the question, you will also be able to add a minimum and maximum date that can be input.



Once you have input the data required ibn the configurations, select Save.


To Create Groups:


Groups are created as themes or titles for different sections in a survey you are creating, so you can group and organise different questions together.


Select Groups from the side menu, under 'Item Surveys'. This will take you to a screen with a list of all Groups under an Active and All tab - same as mentioned above for Questions.



To create a new Group, select Add. This opens a template group pop-up to fill in the details for the Group,

  • Name
  • Instructions - description of the Group shown to the user filling out the survey
  • Display Type
    • Hint - instructions will show when mouse hovers over the information icon
    • Note - instructions will show under the question
  • Multiple Occurrence - input the minimum number of times the group can be used in a survey
  • Active/Inactive switch
  • Description - description of the Group viewed by you and club administrators, will not be shown to user filling out the survey


Once all the details have been filled out, select Save.



Creating Survey Templates:


Templates are pre-defined sets of Groups and Questions that are created for the purpose of being reused on different items.


Select Templates from the side menu, under 'Item Surveys'. This will take you to a screen with a list of all Templates under an Active and All tab - same as mentioned above for Questions and Groups.



To create a new template, select Add. This opens an Item Survey Template pop-up where you will need to fill in the details for the Template under the About tab,

  • Name
  • Description
  • Active/Inactive switch

Once these details have been filled in, select Save.



You will then be able to navigate to the 'Questions' tab, select Add to input Questions and Groups that you have already created.



Once you have input all the Groups and Questions, they are displayed as a list, with the following columns,

  • Position - shows the order in how the questions appear
  • Questions
  • Group
  • Multiple Occurrence
  • Data Type
  • Mandatory
  • Active


To assign a survey to an item:


Note: You cannot assign a full survey template to an already created item, a survey template needs to be added when the item is first created. 


However, you can add questions to an item that has already been created,

  • Open the item and select the Survey tab
  • Select the Add button
  • This opens a Survey pop-up window where you can select the details you want to include
    • Select the survey question
    • Select the template group
  • Press save



The created Item Survey will be displayed to the customer when purchasing the item/adding an item to the cart
